Most likely the electric latch on the cockpit door has failed. After 9/11 all airliners had to have armoured cockpit doors installed that can be only opened from the cockpit and the latch has to be failsafe, ie when it fails it must lock the door. For convenience the pilots are given a push button that unlocks the door when the flight attendant brings them coffee. When it fails the sidewindows or the cockpit hatch are the only way out. It is if you have the money and/or time and skill. Here in the Great White North the strut turrets frequently rust out. You can buy the sheet metal stamping, drill out the spot welds to remove the old one and weld the new one in. Doing so requires a lot of disassembly and reassembly so in most cases it's more economical to scrap the car. Al_in_Ottawa

First thought on #1 was that it probably gave up over time and made some noise, but I'm not seeing any rust in the failure. Can it be repaired? Yeah, but doing it right is going to be spendy. Or,buy the part, cut the top of the tower off ,down far enough to splice and reinforce it, mehh, it would work, but a look under the hood would let anyone with eyes know.
I think #1 is one of the shiny new all aluminum F-150's. Aluminum work hardens WAY faster than steel, and that is a prime spot for that, having already undergone all the stress of a deep stamping to start with.
