It looks as though he drove completely past the king pin and was pulling it with the front of the 5th wheel until he turned and it slid off to the right.
#2 I'm wary of ladders, even Type A... I hate having to climb like 12' up and try to drill through metal with no safety equipment but business requires risks and sacrifices. Even more fun is when I screw several pallets together, then screw the ladder down to a stack of pallets that are dubiously level.

Even for ladder and roof and tree trimming work around the house, it is worth buying a safety harness (~$30 for a minimally adequate one--with leg straps). But the fall arresting devices (fall limiters) are expensive with the cheapest adequate ones around $200. In any event, it is worth $230 to ensure that you will not end up in the hospital from a fall, as rare as those events are. The problem is training yourself to use it, and not just say, I'm climbing this ladder just this time....
The very best accessory to have for any extension ladder is a safety arm / standoff arm to go on the top end. It gives great stability, the ladder won't slip, and it stands you away from the wall about a foot, which is much better for working.
Bingo Arc. I hate ladders. I spent three quarters of my life climbing power poles and working high voltage (with rubber gloves and hot sticks) and I'm scared to shit when I have to climb a ladder. Would rather be in my tools.
Saw a video from China or somewhere out in that vicinity, where a women and child were coming to the top and something happened to the first grate where you step off the moving escalator. The woman was able to throw the child clear but she was eaten by the moving stairs and disappeared below. Nasty. MadMarlin
#3: I've got an XJ. Stock there are no front hard points for towing/tie down. I installed a Curtis front receiver for just that reason. It bolts on using the front sway bar clamp points and can accommodate a winch.
