Looks like some shabby cement construction - the stairs held together but the whole thing de-attached from the wall and fell. Wonder if anyone was on it, since that guy was already there looking over the edge, and they were obviously painting....
I was raised on a horse farm, and #3 is precisely the reason I stayed off hunters and jumpers. Like Anonymous CC said, it's how Chris Reeve broke his neck - and how numerous other people have been seriously injured.
