Maybe, but it looks to me more like a mechanical failure. Front brake locked, rider did an endo. Either that or an inexperienced rider tried to shift and yanked the wrong lever. Bike's controls aren't as standardized as cars, so it might just be somebody riding a bike they're not used to too.
Coal dumper kind of reminds me of the hydraulic ramp that empties wood chips at the paper mill. Semi drives on ramp, ramp raises up and the chips slide out of the back of the 53' trailer.
The mill in Camas had a chip truck dumper, too, and about every two years a truck would come off the ramp because it hadn't been properly secured. We got rail deliveries as well, but those cars weren't dumped, they were vacuumed -- which I thought was a kind of cool.
Oh man... I remember as a kid, when the wind came OUT of The Gorge, the STANK we had to deal with! Almost as bad as the smell from the Lucky Lager Brewery downtown. (Bonafieds: I remember when the ONLY things on 164th were a church, a farmhouse, and an Elementary school where I attended pre-school)
Wouldn't #4 create a void under the one tile, guaranteeing it'll crack at some point in the future? I would rather peel the one tile back up and rework the underlayer material.

#2 makes sense, kind of. Water pressure out of the hose can blast the dirt right out of a container. The water bucket doesnt.
